1. Complete the Ship To information below
2. Click on "Produce Order Form" button
3. Verify all order information, including shipping (if outside the Continental US or Canada, please Email or call for shipping fees) and quantity ordered.
4. Shipping is via U.S. Postal 4th Class, 3 weeks for delivery. Rates (US) are $7 per book for single book orders and $18 per case for full case orders (13 books), (Canada $10/book and $50 case). Please Email or call for International shipping fees.
5. Faster shipping is available upon request please ask for rates. (IE: 2 Day Air, Federal Express)
6. No shipments will be made mid-June through July
7. We do have quantity discounts for full case (13 book) orders which reflect a 10% discount ($98.95).
8. Use your Browser's Print function to print a copy of the Order Form
9. Include a check or Purchase Order for the amount listed and mail it and the Order form to:
